Health Services, NHS Services, Adult Social Care
Applies to
  • Clinical Commissioning Groups or others considering development of EPaCCS to improve end of life care provision/supporting choice
  • Primary, secondary and community care teams providing end of life care to adults (aged 18 or over)
  • Social care providers, ambulance services and out of hours services providing end of life care to adults (aged 18 or over)
  • Those pharmacies, voluntary sector and private sector organisations who provide end of life care to adults (aged 18 or over)
  • System suppliers to the providers and commissioners above
  • Providers of systems contributing to, or holding information relating to, care co-ordination for adults at the end of their life
