| Medication | Details of continuation / addition / amendment / discontinuation of medications. |

| Referral | A request for a care service, other than a specific diagnostic investigation or diagnostic procedure, to be provided for a Subject of care. This includes Subject of care self-referrals for an appointment to see or be in contact with a Care Worker of an Organisation. If there is a verbal request this would normally be confirmed by written request, and these should be processed as one referral. An electronic message is treated as a written referral |
| Service | Service provided to support the Subject of care. Services may include health and social care services (for example, GP services, clinical psychology and occupational therapy), and other services such as cleaning, catering, gardening, transport, education, learning or activities. |

| Transfer of care | When you move from one place of care to another, such as from hospital to your home, supported housing or residential care. Your transfer should be properly planned and coordinated, and health and social care services should work together. Transfers of care may be delayed for various reasons. For example, you may be ready to leave hospital but end up staying there longer than you need to, while you wait for community care services or a place in a care home to be available. |
