The Model Community Health productivity metrics for community health services delivered in the community includes metrics such as the proportion of clinicians’ time spent with patients in individual services.

All NHS trusts that submit five or more services to the Community Services Data Set (DCB1069) are requested to submit to this collection.

Data will be published on NHS England's secure Model Community Health website to help trusts identify opportunities for improvement.
