NHS England SNOMED CT browser

An international standard for clinical terminology published by SNOMED International and providing a global common language for health terms.

SNOMED CT was published as an information standard by the Information Standards Board in August 2011; this version details amendments to both the published standard and the dates for implementation.
SNOMED CT is an international clinical terminology that provides the vocabulary for IT systems to support the direct management of the health and care of an individual.
It also enables the representation of clinically relevant information consistently and reliably in a way that can be processed by IT systems. This enables IT systems to exchange data across the health and care environment; provide clinical decision support tools and undertake enhanced analytics to support effective delivery of high quality healthcare to individual people and populations.
SNOMED CT is required to be used for communicating clinical content across health and care within the <a href="https://www.england.nhs.uk/wp-content/uploads/2015/03/item9-board-260315.pdf" target="_blank">NHS standard contracts</a> and is also stated as the required terminology to support direct management of care within the policy document published by the National Information Board (NIB): <a href="https://www.gov.uk/government/uploads/system/uploads/attachment_data/file/384650/NIB_Report.pdf">Personalised Health and Care 2020: A Framework for Action</a>.
About this change
SNOMED CT is the only current information standard for clinical terminology and this change seeks to ensure its implementation is planned to coincide with national requirements in relation to electronic records. There are three elements to the change:
<ol>
<li>Confirmation of the timescales for the cessation of SNOMED CT releases in Release Format 1 (RF1) and the subsequent move to releases only being provided in Release Format 2 (RF2). All existing users (including any social care providers) will need to change to RF2.</li>
<li>Conversion of the current information standard to align with the requirements of the Health and Social Care Act 2012. This includes the provision of Implementation Guidance.</li>
<li>Confirmation of application of this standard in specified operational settings including implementation dates in alignment with national policy.</li>
</ol>
Existing users need to be aware that SNOMED CT will be required to be incorporated into any future information standards that require consistent recording and interoperability of clinically related data items.
Implementation dates
All Current users (including any social care users) of SNOMED CT in Release Format 1 (RF1) must move to SNOMED CT in Release Format 2 (RF2) no later than 1 October 2018.
Providers of health and care are required to be paperless at the point of care before 2020: supporting IT systems must incorporate SNOMED CT as the clinical terminology.
The following is a summary of conformance dates for appropriate implementation of SNOMED CT that all providers and standards developers must be aware of when planning new, or making changes to existing IT systems or relevant operational information standards:
<ul>
<li>Systems used by, or communicating coded clinical data to, General Practice service providers must use SNOMED CT as the clinical terminology within the system before 1 April 2018. SNOMED CT must be utilised in place of the READ codes before 1 April 2018.</li>
<li>Systems used within Secondary Care, Acute Care, Mental Health Services, Community Services, Dentistry and Optometry - for the direct management of care of an individual - must use SNOMED CT as the clinical terminology standard within all electronic patient level recording and communications before 1 April 2020.</li>
<li>Systems used by all other providers of health related services where the flow of information for the direct management of patient care comes into the NHS must use SNOMED CT by 1 April 2020.</li>
</ul>
Mandated dates for implementation in social care are expected to be part of a future update of the standard.

This information standard is published under section 250 of the Health and Social Care Act 2012

Underpins the development, maintenance and publication of the UK Core Fast Healthcare Interoperability Resources (FHIR) standard.

This information standard formally introduces the governance process for the oversight, direction and leadership of the UK Core Fast Healthcare Interoperability Resources (FHIR) Release 4. The standard mandates, within the health and care sector in England, a consistent approach to the development of UK Core resources, thereby facilitating clear interoperability in the sharing of data across the NHS, health services and adult social care.
There is no set conformance date for this information standard but those in scope of the standard (as set out above) must now determine its impact and
make plans to address that impact.

This standard covers NHS employment lifecycle from being a candidate for a post in the NHS through appointment, onboarding, job planning, payment, rostering, professional development, to leaving for another role or retirement. It aims to support the NHS People Digital Strategy that follows from the NHS People Plan, and defines the vision and benefits for interoperable workforce management tools and information. A key objective is to enable a joined-up workforce systems to provide staff with better operational tools and to provide managers with meaningful data for planning and to improve services.

## Standard definition

This information standard aims to make sure that people who have a disability, impairment or sensory loss get appropriate information and communication support from NHS and adult social care services.

 It requires all applicable organisations to identify, record, flag, share and meet the information and communication support needs of patients, service users, carers and parents with a disability, impairment or sensory loss.

Successful implementation will lead to improved outcomes and experiences, and the provision of safer and more personalised care and services to those individuals who come within the scope of the standard.

## About this change

The standard has been updated following a post implementation review undertaken in early 2017. Changes to the standard include:
* Updates to the meaning of certain terms used, including changes to the definitions for ‘parent’ and ‘highly visible’. 
* Amendments relating to mental health service users and the Mental Capacity Act 2005, to improve clarity, especially regarding support for people who may lack capacity.
* Inclusion of advice regarding support for individuals with a learning difficulty.
* Updates to reflect the current position regarding the inclusion and transfer of data recorded using existing systems.

The revised Implementation Guidance reflects these updates and also includes additional guidance on sharing of information (including consent), use of email / text message to communicate with patients / service users, parental responsibility, and to address some other specific queries that have been raised about interpretation. There are no substantive amendments to the Requirements of the Standard and no substantive changes to the overall scope.

Applies to: This standard applies to – and therefore must be implemented and adhered to by – ALL providers of publicly-funded NHS, public health and adult social care services.

Effective implementation will require such organisations to make changes to policy, procedure, human behaviour and, where applicable, electronic systems

Impacts Upon: Commissioners must ensure that their commissioning and procurement arrangements with providers of health and / or adult social care reflect, enable and support implementation and compliance with this standard. They must also seek assurance from providers of their compliance with this standard.

Implementation of this standard impacts all suppliers providing systems to the above providers; suppliers should work with their customers to determine appropriate use

Legal authority This information standard is published under section 250 of the Health and Social Care Act 2012.

It supports compliance with obligations under the Equality Act 2010.

* some additional features within the filter section of the Current standards page may be beneficial for some blind users, such as the option to reset the filter options
* the details component used in standard listing pages is problematic for VoiceOver users
* the feedback link below the main menu does not include the destination as part of the hypertext
* when Window’s default colour inversion is used the search input is difficult to decipher for users with low vision
* nested lists in some of our content pages can make reading slow and difficult to some screen readers
* links to websites or software that we do not own or manage and so cannot guarantee their accessibility

#### Understanding how social networks use your data

Social networks use information about your online activity to build a profile of you. This data is then used (anonymously) to send you targeted adverts across various digital platforms.

You should be aware that interacting with accounts such as ours may help build the profile of you that social networks maintain, and that could potentially result in you receiving adverts about related issues.

This process of collecting data for advertising purposes is not controlled by the NHS Standards Directory, and we do not have access to the profiling data stored by social networks about you.


## Keeping your personal data secure

We convert your data into secure code (encrypt it) and store it on secure servers in England. A partner organisation is providing hosting services but has no say in how the information is used. There are no legal ways for their employees to see the data. Only approved people in the NHS Standards Directory team can see it.

If you shared your email with us as part of a survey, we will delete it after 2 years. At that point no one can identify you in the survey data.

NHS England may share anonymous information on how the service is used internally.

## What this service does

The directory is a source of recognised standards used by technologies in health and adult social care to exchange data in England. 

It is a signposting service only. It doesn’t include the full documentation for each standard. This can be found on the standard owner’s website.

## Who this service is for

This service is for anyone building or buying digital technologies for the NHS or local authority-funded adult social care services in England. 

This includes IT suppliers, healthcare providers and technology buyers such as procurement teams and framework operators.

## What standards are included

The directory lists all standards that are mandated at a national level as well as some standards that are not mandatory but are widely in use in England. These include: 

* all approved national [information standards](https://digital.nhs.uk/data-and-information/information-standards/information-standards-and-data-collections-including-extractions/publications-and-notifications/standards-and-collections#approved-standards-and-collections). These are assured by the [Data Alliance Partnership Board](https://digital.nhs.uk/data-and-information/information-standards/information-standards-and-data-collections-including-extractions/data-alliance-partnership-board) and issued with an Information Standard Notice, which means they are mandatory.
* APIs published in [NHS Digital's API Catalogue](https://digital.nhs.uk/developer/api-catalogue)
* record standards published by the [Professional Record Standards Body](https://theprsb.org/)

It also includes draft APIs and draft standards that are in the process of being assured at the national level.

## Benefits of using standards
There are many benefits to using standards including:

* it’s more efficient to reuse existing approaches to recording or exchanging data than creating new ones
* it promotes quality by ensuring all care settings are aligned to standards that have been approved by qualified bodies
* it makes it easier to analyse data at the national level and see trends or problems if the data has all been standardised 
* it is essential to achieving interoperability, which would allow IT systems across the country to share data. An example of this is hospitals being able to update GP records directly using their own systems instead of sending emails

## Report a gap in standards
Work is underway to publish a process for people developing new standards. 

In the meantime, please notify us of plans to create new standards by emailing england.interop.standards@nhs.net. Local standards that apply at the national level may be uplifted for national use.
