Adult Social Care Digital Social Care Record: Minimum Operational Data Standard (MODS)Person demographics
Details of the person's demographics. It is advisable to use the Personal Demographics Service (PDS) for this purpose. The PDS can be used to verify and employ essential patient demographic information, including but not limited to the patient's name, address, date of birth, contact details, their registered general practitioner (GP), and preferred pharmacy. It also provides a national unique identifier for each patient, known as the NHS number. For further details, refer to the digital resources provided by the NHS on their Personal Demographics Service webpage.
