More information

Unplanned hospital admissions were hoped to be avoided by improving services for vulnerable patients and those with complex physical or mental health needs who were at high risk of admission or re-admission.

This was discontinued as it was removed from the General Medical Service (GMS) contract on 31 March 2017.
