NHS Continuing Healthcare (NHS CHC) means a package of ongoing care that is arranged and funded solely by the NHS where the individual has been assessed and found to have a ‘primary health need’ as set out in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing.

This new standard, the NHS Continuing Healthcare (NHS CHC) Patient-Level Data Set, sets out the rules for an end to end, patient level data set that captures data relating to:

  • Patient details (date of birth, gender, etc.)
  • NHS CHC eligibility (checklist outcome, Decision Support Tool (DST) outcomes, eligibility, decision process, referral-to-decision times and backlog, etc.)
  • NHS CHC activity (numbers eligible, newly eligible, no longer eligible etc.)
  • Annual, 3-month and ad hoc reviews
  • Local requests for review of eligibility decisions.
  • NHS CHC care packages

Data is submitted monthly by data providers to NHS Digital via the Strategic Data Collection Service (SDCS) Cloud service.
